You will manage a caseload of 15–20 participants, serving as their central point of contact.Conduct initial and quarterly psychosocial assessments.Collaborate with participants on individualized service plans and housing stability goals.Build trusted, professional relationships with clients.Provide outreach, housing navigation, and ongoing support.Facilitate referrals to healthcare, mental health, substance use, and community services.Support clients in obtaining employment, benefits, and education opportunities.Accompany clients to key appointments as needed.Use a harm reduction, strength-based, and client-focused approach.We are deeply invested in staff development. This role provides a clear advancement pathway—from Case Manager to Lead Case Manager and ultimately to Program Manager.
